The honest verdict: the absorption multipliers printed on curcumin bottles are real numbers from real studies, and nearly every one comes from a study funded by the manufacturer of the product that won. When an independently funded group measured the compound that actually matters, free unconjugated curcumin, one popular "optimized" form was below the limit of detection. At the same time, the NIH now classifies turmeric as a well-documented cause of liver injury, and the enhancement ingredient sold to raise absorption is the one regulators specifically implicate. This guide reports what each study measured, who paid for it, and what that leaves you with.
The NIH's LiverTox database assigns turmeric a likelihood score of "A": recently established as a well-documented cause of clinically apparent liver injury. That is its highest causality category.
In the NIH-funded Drug-Induced Liver Injury Network series, 10 turmeric cases were identified, all enrolled from 2011 onward and six since 2017. Nine of ten were hepatocellular, five were hospitalised, and one died of acute liver failure. Median time to onset was 86 days. Seven of the ten carried the HLA-B*35:01 genetic variant, against roughly 6% of controls.
Piperine is specifically implicated. LiverTox states that bioavailability enhancement using piperine was "subsequently linked to several cases of liver injury," and Australia's TGA concluded in 2023 that the risk is "potentially higher for products with enhanced absorption or bioavailability." NIH NCCIH puts it plainly: conventionally formulated turmeric "is likely safe in the recommended amounts for up to 2 or 3 months," while "highly bioavailable formulations of curcumin ... may harm your liver." Stop and seek medical advice if you develop jaundice, dark urine, unusual fatigue, nausea or abdominal pain.

Avoid if you have any liver condition, drink heavily, or take a medication with hepatic risk. Turmeric supplementation is a documented cause of liver injury, and supplement use is not the same as cooking with the spice.
Treat piperine-enhanced products with extra caution: they are the class regulators single out, and piperine independently interferes with the metabolism of many prescription drugs.
Discount the multiplier on the label: 45x, 39x and 29x claims all come from manufacturer-funded studies measuring mostly inactive metabolites, against a comparator dosed nearly five times higher.
What the evidence supports: per NCCIH, initial positive evidence for osteoarthritis, and not enough evidence to conclude a benefit for anything else.
How much curcumin actually reaches your blood?
Curcumin absorption is genuinely poor, but the widely repeated "approximately 1% bioavailability" figure does not trace to any human measurement we could find. The paper usually cited for it is a review that contains no such number. We previously published that 1% figure ourselves and have removed it, because the measured human data is both more specific and more striking.
In a publicly funded dose-escalation study of standardised curcumin, serum curcumin was undetectable at 500, 1,000, 2,000, 4,000, 6,000 and 8,000 mg. Only 2 of 24 subjects had detectable serum at all, at 10,000 and 12,000 mg. A Phase I trial found curcumin and its metabolites "in the 10 nmol/L range," and an 8 g/day trial measured plasma peaks of 22 to 41 ng/mL.
The reason this matters for what you buy is that most of what circulates is not curcumin. It is glucuronide and sulfate conjugates, which are poorly membrane-permeable. A US Pharmacopeia expert committee review states that systemic exposure "is dominated by glucuronide and sulfate conjugates, while free curcumin remains minimal and transient despite formulation strategies or adjuvants such as piperine."
Who funded the absorption studies behind each multiplier?
Every headline multiplier in this category traces to a specific study, and the pattern is consistent enough to be the most useful thing on this page.

Two details are omitted from every consumer-facing version of these numbers. First, in the two studies we could fully verify, the branded formulations were dosed at 376 mg of curcuminoids against an unformulated comparator at 1,800 mg, a 4.8-fold dose disadvantage for the thing being beaten. Second, both papers declare "no competing interests" in the same document that names the winning product's manufacturer as the funder, and two authors appear on both, with a different sponsor's product winning each time.
The independent measurement changes the picture. A group at Amsterdam UMC, funded internally with no declared competing interests, measured free unconjugated curcumin across five interventions in healthy men. A standard extract was below the 2 nM limit of detection. Longvida was also below the limit of detection. Only NovaSOL produced detectable free curcumin, at a median peak of 14.9 nM, while its conjugated curcumin measured roughly a hundred times higher. The authors argue directly that "bioavailability claims should be based on unconjugated curcumin and not on its poorly membrane permeable conjugates," and attribute the large published multipliers to measurement after enzymatic hydrolysis in studies sponsored by the same companies.
A separate methodological audit of 171 systematic reviews of oral curcumin found that bioavailability appeared in the methods of only 9.3% of them. Its conclusion was that most systematic reviews on orally administered curcumin "should be taken with caution."
Does curcumin actually work?
The most authoritative neutral assessment is NCCIH's, and it is more restrained than this category's marketing. Its conclusion, verbatim: "We don't know enough to definitively conclude if turmeric or curcumin is beneficial for any health purposes."
On osteoarthritis it says the initial evidence is positive and higher-quality evidence is needed for a definitive conclusion. On non-alcoholic fatty liver disease, initial research suggests possible improvement in some measures, though which measures improve consistently is unclear. For everything else, there is not enough evidence to reach a conclusion. We previously described curcumin as "one of the most evidence-backed anti-inflammatory botanicals available." That overstated the position of the neutral authorities and we have corrected it.
If you are going to take it, what should you take?
These are ranked on the strength and independence of the evidence behind them, not on the multiplier printed on the bottle.
Is turmeric itself contaminated?
Culinary turmeric has a documented lead-adulteration problem that is distinct from the supplement question. Stanford-affiliated researchers surveyed 524 samples across 11 districts of Bangladesh and found lead chromate pigment added to brighten turmeric in seven of nine major producing districts, with turmeric lead concentrations reaching 1,152 micrograms per gram, hundreds of times the national legal limit. Yellow pigments ran 2 to 10% lead by weight.
This is a supply-chain risk concentrated in imported culinary turmeric rather than a documented problem with US supplement extracts, and we could not verify current FDA import-alert status for turmeric specifically. It is a reason to buy standardised extracts from brands that publish testing, rather than bulk culinary powder, if you are consuming turmeric daily.
Other safety considerations and drug interactions
Piperine and prescription drugs. Piperine inhibits CYP3A4 and CYP2D6, the liver enzymes that metabolise many prescription medications including warfarin, SSRIs, statins and blood-pressure drugs. If you take any prescription medication, choose a piperine-free form or speak to your doctor first.
Surgery and blood thinning. Curcumin has mild anticoagulant properties. Stop at least two weeks before any scheduled surgery.
Gallbladder. Curcumin stimulates bile production. Avoid it if you have gallstones or bile duct obstruction.
Pregnancy. NCCIH states that the use of turmeric supplements during pregnancy may be unsafe.
Duration. NCCIH's safety statement covers conventionally formulated turmeric for up to two or three months. Indefinite daily use at high doses is not the exposure any of the safety data describes.
The bottom line
Curcumin sits in an unusual position: a genuinely interesting compound whose consumer market has been built on a measurement artifact. The multipliers are technically accurate about total curcuminoids after hydrolysis, and largely meaningless about the free compound that has to reach tissue to do anything.
Meanwhile the enhancement most aggressively marketed, piperine, is the one the NIH and Australia's regulator connect to liver injury. The plausible reading is that a buyer of the premium feature is taking on elevated hepatic risk without a matching increase in active-compound exposure.
If you want to take curcumin, a piperine-free phytosome at a conventional dose for a defined period is the defensible choice, and stopping if any sign of liver trouble appears is not an abundance of caution but the specific thing the case series describes. If you want a well-evidenced anti-inflammatory intervention, the honest answer is that omega-3 has substantially more independent human evidence behind it than curcumin does.
